CMS's Cascade of Meaningful Measures identifies Closing Gaps in Care as a quality-measurement priority, not as one universal measure or workflow. CMS's 2026 eCQM resources show that electronic quality measures are versioned, program-specific specifications with defined logic, data elements, value sets, and implementation guidance, and that a candidate measure is not reportable until finalized for the applicable program. ONC describes alerts, reminders, summaries, and other clinical decision support as tools that can present timely, person-specific information. Together these sources support a distinction between identifying a possible gap, completing clinically appropriate care, capturing valid evidence, and satisfying a specific reporting program. They do not validate a vendor's gap list, recommend care for an individual, or establish payer credit. Buyers must verify measure version, attribution, source freshness, exclusions, data lineage, clinician review, outreach and consent, equity impact, closure evidence, and current program rules.
